1. Liability Release: Positive Interactions Dog Behavior and Training, LLC will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Positive Interactions Dog Behavior and Training, LLC is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Positive Interactions Dog Behavior and Training, LLC will be responsible for any such acts or omissions, but only to the extent of such insurance. I have been told by Positive Interactions Dog Behavior and Training, LLC and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I represent and warrant that I have provided Positive Interactions Dog Behavior and Training, LLC with full and complete, accurate information regarding any bite history and similar incidents or hazardous tendencies of my dog, and that I will update that information if it changes. I understand that I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify, release, and hold harmless Positive Interactions Dog Behavior and Training, LLC of any and all claims, whether made by myself or any third party, of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service. This contract, together with language expressly incorporated into it in writing, is the full and complete agreement between me and Positive Interactions Dog Behavior and Training, LLC. A complete and accurate copy of this contract is as valid as the original. This contract is made valid by in-person signatures, electronically signed signatures, or upon receipt of a signed, scanned copy by email.

4. I authorize emergency medical care to be provided for my dog(s) by the above-named veterinarian, or an appropriate alternate to be determined by Positive Interactions Dog Behavior and Training, LLC in the event that my regular veterinarian is not available or that closer care is required. I will reimburse Positive Interactions Dog Behavior and Training, LLC for any charges related to emergency care, including office visits, procedures, medications, surgeries, etc.

5. Service Policy: I understand that Positive Interactions Dog Behavior and Training, LLC will work directly with myself and my dog(s) to communicate current animal behavior knowledge that best fits needs, and that successful completion of a behavior modification plan depends on my consistency with training. I understand that behavior is fluid, and my dog(s) will not continue to perform even previously trained behaviors without ongoing practice. Especially in cases involving any type of aggression, although behavior may be modified, I understand dog(s) are never considered “cured.” My dog(s)’ behavior and/or interaction with humans and other animals is ultimately my responsibility. Positive Interactions Dog Behavior and Training, LLC will make every reasonable effort to help me in attaining agreed upon goals but makes no guarantees of performance on the part of myself or my dog(s). I will assume full responsibility for keeping and/or rescheduling appointments, on-going communication, and practice. I understand that Positive Interactions Dog Behavior and Training, LLC reserves the right to transfer in-person dog training services to online training services when deemed necessary by Positive Interactions Dog Behavior and Training, LLC for purposes of compliance with government regulations or considerations of client or trainer/consultant. Such changes in the delivery of services is not ground for postponement; postponing services may result in their being forfeited.

6. Payment and Cancellation Policy: Consultation and individual training session fees are due in full at completion of consult or training session. Package fees are due and payable in full at completion of the first training session. All package sessions must be used within 3 months of initial start date after which time, no refunds will be given but any remaining balance can be applied as credit for future training. I acknowledge that consistency is key to my dog’s training success and I will choose my consulting and training dates carefully. Missed consultations or training sessions will be charged Positive Interactions Dog Behavior and Training, LLC’s hourly rate unless agreed upon by both Positive Interactions Dog Behavior and Training, LLC and myself. Twenty miles’ travel each way from Consultant’s home to my home (or other training location) is included in all in-home appointments. Locations beyond 20 miles may accrue a travel fee per visit, roundtrip. I understand that changes in service due to the Coronavirus are not grounds for cancellation, and that Positive Interactions Dog Behavior and Training, LLC reserves the right to change the mode of service delivery (for example, from in-person to online) as deemed necessary and responsible by Positive Interactions Dog Behavior and Training, LLC.

7. Safety Policy:  I warrant and represent that I will remain aware of and abide by local law and public guidance on reducing COVID-19 exposure and will follow best practices in relation to Positive Interactions Dog Behavior and Training, LLC services regarding social distancing and sanitation, including the wearing of a mask, maintaining six feet of social distance. I understand that I may be asked to confirm my health and the health of all other members of my household, including awareness of any fevers or other symptoms, before each in-person training session. I represent and warrant that I will provide full and complete, accurate information as known to me. For all outdoor training, I agree to have my dog in an appropriately fitted flat, buckle collar or harness along with a sturdy, 6 feet leash. I agree to not use a choke collar, prong collar, electronic collar, vibration collar or retractable leash during training sessions. The use of these types of training equipment are not used or recommended by Positive Interactions Dog Behavior and Training, LLC.

This contract is validated by the signatures below in total and as approval for future services without additional written authorization.
